We are seeking an Advanced Practitioner to join our client's innovative team in the Washington State that is committed to improving care for underserved and complex-needs patients. You will be based in one of our 3 care centers (Lynnwood, Renton, or Lakewood), but will include travel to all of our WA-based care centers based on patient and team needs.

The key to our care excellent care model is relationship based care, therefore our schedules allow providers to spend an extended amount of time with our patients. You are supported by our amazing robust clinical care teams consisting of a medical assistant, primary care nurse & patient services representative. You will also partner with our multi-disciplinary team (nurse care manager, clinical social worker, clinical pharmacist, and mental health specialist) to bring closer attention to the social determinants of health, and an intense focus on transitions of care, always seeking to identify root etiologies for avoidable utilization of the health care system. You will spend 20% of your clinical time on administrative duties and 80% of your time providing excellent patient care. Our engagements typically range from 10-12 patients per day and spending between 30-60 minutes with each one.

  • Graduation from an accredited health professions school
  • Board Certified Physician Assistant or Nurse Practitioner
  • Active Medical License in the State of Washington, or you would have the ability to obtain one.
  • Ideally, you will have three or more years’ experience since completion of school
